The Rapid Access to Social Care Team (RASC) supports patients to be discharged sooner from hospital or avoid hospital admission in a crisis situation by providing a short-term care package up to four times a day. This social care is provided and continued until social services can provide a care package to suit the patient’s needs. RASC provide personal care and assistance to patients in their own home and can also support with administration of medication. Referrals are made via Care Coordination on 01902443322.

Service operates: 8.00am – 10.00pm, 7 days a week.
