Who we are


The RWT Community Anticoagulation Service is a nurse led service with clinical oversight from a Consultant Haematologist. The team is based at Pendeford Health Centre operating from other venues across the city Monday to Saturday (8:30am – 4:30pm). The service also offers a domiciliary service for housebound patients.

Our Aims


All patients who require oral anticoagulation therapy will receive a safe and comprehensive clinical service. All patients who are started on an anticoagulant or are switched to an alternative anticoagulant (i.e., from Warfarin to a DOAC) should be referred to the community anticoagulation service to ensure that they receive appropriate assessment, education, and monitoring of their treatment. We currently have approximately 3,500 patients on our caseload which includes those requiring domiciliary visits.

What we offer


  • Nurse led clinics for new patients

  • In reach support service to acute wards to facilitate safe anticoagulation management and preparation for discharge
  • VTE Audit support
  • Lead support from Consultant Haematologist for complex case management
  • Complex caseload co-ordination/management via the Senior Sister.
  • Nurse prescribing
  • Comprehensive education package for all new/ switch patients
  • Nurse dosing line which includes dosing support for patient’s ringing in with their INR results
  • Telephone dosing line for all new Cannock patients until their treatment is stable via Cannock Single Point of Access.
  • Admission avoidance by administration of Vitamin K/ Low Molecular Weight Heparin (LMWH) for patients who require it

How to get a referral


  • All patients registered with a Wolverhampton GP who are newly commenced on Anticoagulation treatment OR switched to an alternative Anticoagulant
  • Aged 18 years or above
  • Limited service offered to complex patients with Cannock GP’s (referral via Care coordination and Cannock SPA)
  • Home visit patients to live within the city of Wolverhampton

Exclusions


  • Patients under 18 years
  • Patients who are NOT registered with a Wolverhampton or Cannock GP
  • Patient of a GP operating an enhanced service scheme to manage their own anticoagulation service (WUCTAS can advise on referral)
