Who we are


As a Community Urgent Care team, we are focused on improving the life chances and health outcomes of the diverse population of the communities we serve. We are passionate about supporting our patients and endeavor to enable them to remain at home when they are having a crisis, both of a clinical or social nature. Our admission avoidance service is nurse led with a team of highly skilled nurses and Advanced Clinical Practitioners (ACP’s) with medical Consultant cover five days a week. This gives us access to a wide range of interventions and treatments such as requesting blood tests, x-rays and ultrasounds. We have nurse prescribers available 24 hours a day, 7 days a week. By understanding the needs of our communities, we can use our clinical expertise to significantly contribute to the development of pathways that continually improve the quality of care.

There are three teams that sit within Community Urgent Care:

A nurse led rapid response service that includes Advanced Nurse Practitioners, Specialist Nurse Practitioners (both non-medical nurse prescribers) and Specialist Nurses that are supported by a medical Consultant from the Care of the Older Adults Directorate. The service provides assessment, diagnostics, and treatment to residents of nursing homes, residential homes, and adult patients in their own homes within Wolverhampton where an admission to acute hospital would be the alternative. The service also supports with end-of-life planning to enable patients to die in their preferred place of care.

Service operates: 24/7 – 7 days a week.

A nurse led service which operates every day of the year between 8pm and 8am. This service visits clinically housebound patients providing care in their own home, and this includes sheltered accommodation or residential care. They provide central line care, PEG care and feeding, end of life/ palliative care and catheter care.

Service operates: 8.00pm – 8.00am, 7 days a week.

A multidisciplinary team whose purpose is to provide support following hospital discharge or to enable people to remain at home to avoid hospital or social care admission. The main aims are to facilitate independence, provide rehabilitation, enable early discharge, and promote recovery at home. The team consists of senior nurses, staff nurses, physiotherapists, occupational therapists, senior support workers and rehabilitation assistants.

Service operates: 8.00am – 6.00pm, 7 days a week.
