Who we are


The Virtual Ward and Hospital at home is an integrated nurse led service that provides city wide support to patients with acute and chronic illness. Our aim is to facilitate early discharge and enhance admission avoidance by providing treatment and supporting patients in the comfort of their own homes.

The team support patients to use remote monitoring technology where able and appropriate.

How to access the virtual ward/ Hospital at home


Virtual Ward

  • Oximetry monitoring @Home
  • Acute COVID management (step down from acute)
  • Oxygen weaning
  • Covid infusions via COVID Medicines Delivery Unit (CMDU)
  • Infective exacerbation and non-infective exacerbation of COPD
  • Asthma management (infective and non-infective)
  • Acute Respiratory Infection/ Pneumonia
  • Supportive Care (Palliative)
  • Renal
  • Paediatrics
  • Frailty
  • Remote Monitoring in Care Homes
  • Discharge to Scan (diagnostics)

 

Hospital at Home

  • Management of Heart Failure following stabilisation and commencement of sub cut Furosemide
  • IV antibiotics infusion via OPAT Services
  • Management of Liver Abscess following commencement of antibiotics.
  • Management of Hip and Knee infections such as, Osteomyelitis following commencement of antibiotics
  • IV antibiotics therapy for individuals with identified systemic infection (Cellulitis)
  • IV antibiotics therapy for ESBL and Multiple Sclerosis

Our locations and operating hours


The service operates between the hours: 8am – 10pm 7 days per week/365 days a year (last referral 8pm).

Being referred


  • Age 18 and over registered to a Wolverhampton GP
  • Resident of Wolverhampton
  • Not medically fit but able to continue acute level treatment or monitoring at home (must be medically stable)
  • Patient that meets the criteria should be clinically assessed by the referring clinician and diagnosed as appropriate for this service
  • Clinician should discuss referral with patients prior to discharge
  • Identified as suitable via OPAT

The team works alongside other professionals via weekly pathway MDTs as well as other Adult Community Services including: Rapid Intervention Team (RITs), Community Intermediate Care Team (CICT), Ambulatory Care and District Nursing Planned Care.
