Our dedicated team of nurses and health care support workers provide urology support and specialist wound care management for all simple and complex wound care needs liaising where appropriate with our Urology, Tissue Viability and Vascular Services.

• Nurse led clinics for new and existing patients
• Complex care co-ordination/management via the Specialist Nurse Practitioner
• Specialist intervention from diagnosis to healing – with a management plan individually tailored for each patient
• Specialist intervention from Tissue Viability and Vascular Services if required
• Nurse prescribing
• Expert care and advice to patients
• Education and support to patients in self-care promotion
• Access to dedicated venues 7 days a week

Referrals are accepted via Care Coordination (Telephone 01902 443322) or from GP or any Primary or Secondary health care professional and any social care worker

• Aged 18 years or above
• All patients referred into the clinic from both primary and secondary care sources must be able to attend appointments
